Antibiotics

Continue. Give before surgery.

Antiretrovirals

Continue.

Anti-rheumatoid modifying agents (DMARDs)

Seek Rheumatological advice. Methotrexate should be witheld 48 hours pre-op and restarted when renal function is normalised.

Corticosteroids

Continue. Consider if adrenal supression is likely: see section in main contents.

Gastric anti-secretory agents (PPI / H2 antaganists)

Continue

NSAID / COX2i

May be withheld 24 hours pre-op, discuss with anaesthetist. Agents with a longer half like (e.g. piroxicam) may be witheld 3 days pre-op.

HRT / Oral contraceptive

Continue with antithrombotic measures. If cessation required in high risk of thromboembolism, do so for 4-6 weeks with alternative contraception.

Inhaled respiratory agents

Continue. Consider nebulisor if required, or alternate route for corticosteroid.

Theophylline

Withold on day of surgery.

Thyroid and anti-thyroid agents

Continue, give before surgery.
