dabigatran: restarting

If elective surgery with satisfactory haemostasis, restart 1-4 hours postop.

• First dose with a single capsule (75mg/110mg/150mg dependent on indication)

• Then usual daily dose the following day.

A delay restarting may be appropraite depending on surgery, urgency of thromboprophylaxis or if there is significant bleeding.

Do not restart with an epidural or spinal catheter in situ. 

• Delay for at least 6 hours after their removal (longer if traumatic/difficult insertion).
